Please follow the link below to register for our paperless contracting system, SureLC. This program will gather most of your information from the NIPR database, so you just need to fill in your personal/business information and answer the background questions.
Once you’ve completed your profile information and uploaded the required documents such as E&O insurance, voided check copy, electronic signature page, LTC/Annuity CE, etc, you will be able to request the carrier specific appointment electronically by selecting the carrier from the list of options. We will then get an email notification that you have completed the registration process and will submit your request to the carrier. It’s completely paperless each time you contract/appoint with an insurance carrier.
